You also have an option to preview the User Survey form that will appear to the user.
To preview the User Survey form,
Click on the Admin tab in the header pane to open the configuration wizard page.
Click on Survey
Preview icon 
under User
Survey Settings. The Preview Survey page opens with the list of
configured questions and levels.
